About the role:
We are currently seeking a dedicated, enthusiastic and flexible individual to join our team as a Health and Wellbeing Development Officer within the new Trailblazer Project.
The Trailblazer Programme is an ambitious, multi-agency initiative led by Neath Port Talbot Council to reduce economic inactivity and improve wellbeing across our communities. We're currently delivering innovative projects in our Opportunity Hubs and outreach locations, working alongside health professionals, training providers, and employers to help people overcome complex barriers to work. From supporting residents with mental health and long-term health conditions, to tackling digital exclusion, housing instability, and low confidence, we're at the forefront of creating tailored, life-changing solutions that make a genuine difference in people's lives.

As Health and Wellbeing Development Officer you will:
- Lead on the design and delivery of health and wellbeing initiatives that improve physical, mental, and social wellbeing
- Support individuals with long-term conditions or barriers to work through tailored one-to-one and group interventions
- Work with health professionals, local organisations, and employers to coordinate holistic support pathways
- Promote lifestyle improvements, resilience, and confidence so participants can move closer to employment
- Design and deliver workshops, resource packs, and wellbeing events to raise awareness and provide practical support
- Monitor and evaluate impact, using data and feedback to drive continuous improvement
- Conduct health and wellbeing assessments with participants to identify needs and barriers
- Deliver one-to-one support and group sessions on resilience, nutrition, physical activity, and mental health
- Liaise with GPs, mental health services, and community organisations to coordinate support
- Develop action plans with measurable health-related goals linked to employability outcomes
- Organise and promote health campaigns, workshops, and community outreach events
- Record progress, report on outcomes, and share good practice with the team
Do you have:
- Experience in developing and delivering health and wellbeing programmes?
- Strong understanding of the links between health, wellbeing, and employability?
- The ability to support individuals with health conditions or barriers to work?
- Experience working with partners across health, community, or public services?
- Excellent communication, time management, and organisational skills?
- A Level 4 qualification in Advice and Guidance, Health Promotion, or a related field?
